This policy describes how CareOneX, Inc. ("CareOneX", "we", "us") collects, uses, and shares information when you visit careonex.com (the "Site") or submit information through our forms. It applies to the Site only; the CareOneX application at app.careonex.com is governed by the agreements provided to customers within that product.
Information you provide
When you submit a form on the Site (for example, to request a credentialing game plan or a product demo), we collect the information you enter, such as your name, phone number, email address, agency name, and any details you choose to share about your agency's situation. Please do not submit sensitive identifiers (EINs, NPIs, Medicaid IDs) or any patient information through Site forms.
If you submit a request to find home care for yourself or a loved one, we collect the details you provide about that request, such as who needs care, your county, the kind of help you're looking for, how you expect care to be paid for, when care is needed, and your contact information. Please do not include medical details, diagnoses, or other health information in these forms; we do not need them to match you with an agency.
Information collected automatically
- Analytics. We use Google Analytics to understand how visitors use the Site (pages viewed, approximate location, device type). Google Analytics sets cookies and receives your IP address.
- Advertising measurement. We may use advertising tags such as the Meta (Facebook) pixel and Google advertising features to measure ad performance and to show CareOneX ads to people who have visited the Site. These tools set cookies and receive information about your visit.
- Campaign parameters. If you arrive from an ad or email link, campaign identifiers in the URL (such as UTM codes) may be recorded along with any form you submit, so we know which campaign led to your inquiry.
How we use information
- To respond to your inquiry, including preparing and delivering your credentialing game plan and following up by phone or email about payer access, billing, and agency growth.
- To fulfill care requests, including calling you to understand your situation, answering coverage questions, and connecting you with one or more licensed home care agencies in your area.
- To operate, measure, and improve the Site and our marketing.
- To advertise CareOneX, including showing ads to Site visitors and to audiences similar to our contacts on platforms such as Meta and Google.
How we share information
Care requests. If you submit a request to find home care, we share the information in your request with one or more licensed home care agencies in your area so they can contact you about providing care. This is the purpose of the service, and you consent to it when you check the consent box on the form. You agree that CareOneX and the matched agencies may contact you by phone, text message (SMS), or email about your request; message and data rates may apply, and you can opt out of texts at any time by replying STOP or emailing support@careonex.com.
We do not sell your personal information for money. We share it with service providers that help us run the Site and our business, such as Google (analytics, form processing, email) and Meta (advertising), under their respective data processing terms, and with professional advisers or authorities where required by law. Where we use advertising platforms, limited identifiers (such as a hashed email address or cookie data) may be shared to deliver and measure ads; some privacy laws treat this as "targeted advertising", and you may opt out as described below.
Your choices
- Marketing communications. You can ask us to stop contacting you at any time by replying to any of our emails or writing to support@careonex.com.
- Cookies. You can block or delete cookies in your browser settings. Google offers an Analytics opt-out browser add-on, and Meta offers ad preferences controls within Facebook and Instagram.
- Targeted advertising opt-out. To opt out of our use of your information for targeted advertising, email support@careonex.com with "Ad opt-out" in the subject line.
Your rights
Depending on where you live (including under the New Jersey Data Privacy Act), you may have the right to access, correct, or delete personal information we hold about you, and to opt out of targeted advertising. To exercise these rights, email support@careonex.com. We will verify your request and respond within the time required by applicable law. If we decline a request, you may appeal by replying to our response.
Data retention and security
We keep inquiry information for as long as needed to work with you and for reasonable business record-keeping, then delete or anonymize it. We use commercially reasonable safeguards to protect information in our care, though no method of transmission or storage is completely secure.
Children
The Site is intended for adults (home care agencies and families arranging care) and is not directed to children under 13. We do not knowingly collect information from children.
Changes to this policy
We may update this policy from time to time. We will post the updated version here with a revised "Last updated" date.
